Immunization Requirements

All incoming С��Ƶ students are required to submit immunization information. Please access the Student Health web portal to (1) record the dates of your immunizations, (2) upload a copy of your immunization record or Pre-Entrance Immunization Record Form and (3) complete the Tuberculosis Screening Questionnaire.

Incoming students must provide documentation of their immunization record in addition to date entry on the Student Health Portal by submitting one of the following: a copy of the official immunization record or the Pre-Entrance Immunization Record Form.

If you received your immunizations in Maryland, you may have access to your immunization record at the . Immunization records may also be accessed via childhood medical providers and/or public schools.

If you have a medical contraindication or religious exemption to vaccines, please complete the vaccination exemption form and upload it to the secure student health portal.

Failure to complete requirements by, July 31st, for the fall semester or January 15th, for the spring semester will result in a hold on your account that prevents you from registering for classes, and a $100 non-refundable immunization late fee.

Physical examinations are not required for general student admission to С��Ƶ. Athletes have additional requirements to those from Student Health Services; please contact the Athletic Department for questions regarding these requirements.

Required Immunizations

Student Health Services requires new students to submit documentation of the following:

Meningitis (Meningococcal ACWY Vaccine)

Maryland law requires that all students, graduate and undergraduate, enrolled at the University and who reside in campus housing have the Meningococcal ACWY vaccine (Conjugate vaccine, Menactra®, Menveo®, or MenQuadfi®) given at age 16 or older, or a signed waiver on file in Student Health Services.

Student Health Services also recommends Meningococcal vaccine for serogroup B (Bexsero® or Trumenba®) for students residing in campus housing.
Measles, Mumps and Rubella (MMR)
Tetanus-Diphtheria-Pertussis (Tdap)
Tuberculosis Screening

International Students

All international students must report to Student Health Services upon arrival for TB testing, regardless of whether they received the BCG vaccine. A QuantiFERON gold test will be completed in SHS free of charge and must be drawn by the end of the first week of classes. If your QuantiFERON Gold is positive, you must have a chest x-ray. Bring the documentation if you have had a TB blood test performed within 6 months of arriving at С��Ƶ. If the blood test is positive, get the official results in English. If you have ever been treated for a positive TB test or active tuberculosis, bring documentation of drugs and duration of treatment.

Note: Students with a history of a +PPD, negative chest X-ray, and prophylactic INH treatment for 6 to 9 months must provide documented proof. The health department does not require X-rays for these patients with a history of INH treatment. Students should contact their respective departments for further recommendations.

Tuberculosis Test Documentation Form (PDF) - This form is ONLY NEEDED for all international students, and students who answer “YES” to any of the questions on the TB Screening Questionnaire form located online in the Secure web portal.
SHS offers in-house QuantiFERON Gold testing by appointment only on Mondays & Thursdays. The fee for this service is $25. Please call the front desk to make an appt (410-543-6262) or self schedule your appt online via the student health portal.

Recommended Immunizations:

COVID-19: С��Ƶ strongly encourages all members of the campus community to receive the COVID-19 vaccine and all recommended boosters.
Hepatitis B vaccination is strongly recommended and is required for nursing and health majors.
Varicella Vaccine, required for nursing and health majors.
HPV vaccine - Gardasil is strongly recommended (for both women and men)
